Patients are seen by appointment only, so please call in
advance to reserve a time.
We do confirm appointments one to two days prior to the
appointment. If however, you are unable to keep this
reserved time, please notify our office at least 24 hours
prior to the visit during our normal business week of Monday
through Thursday. This courtesy on your part makes it
possible to give the appointment time to another patient.

Payment of Services and Insurance
In order to control the cost of dental treatment, we
request payment at the time service is rendered.
If you have dental insurance, we request you pay the estimated
portion that the insurance will not cover. Again, this portion
is collected at the time of service. Please bring a completed
dental insurance form with you at the initial appointment. As a
service to our patients, we will complete these forms with the
treatment information and submit them daily to the insurance
carrier.
We gladly accept Master Card, Visa, Discover, American Express &
most major Insurances. |
